- Summary: A Home Support Aide is a qualified individual who carries out social, personal and certain health care tasks under the supervision of a registered nurse and who may also provide assistance with housekeeping and other related supportive tasks to a client in his/her place of residence. The aide may perform other tasks permissible under state scope of practice and regulatory guidelines under the direct supervision of a Registered Nurse.
- Essential Duties and Responsibilities: include the following. Other duties may be assigned.
- Assists clients with personal care and activities of daily living, in accordance with the client’s individualized care plan.
- Practices safety, infection control, and standard precautions.
- Maintains client rights and confidentiality.
- Escorts client to appointments.
- Prepares and serves meals according to instructions.
- Keeps the rooms the client uses clean, including making bed, changing bed linens, dusting, vacuuming, washing dishes, tidying kitchen, bedroom, and bathroom.
- Shops or does errands for client.
- Washes client’s laundry.
- Participates in Performance Improvement activities.
- Documents services provided on the appropriate form, and submits the form to the office before leaving the campus.
- Attends in-services, as required.
- Observes Advance Directives status as directed on each individual client. Home Support Aide will receive orientation on Advance Directives from Supervisor.
- Reports to Supervisor any changes in client condition.
- Performs the following permissible Home Support Aide functions under special circumstances, as directed by the supervising RN:
- Takes and records vital signs and/or intake and output.
- Observes, reports, and records client activity.
- Assists with changes of colostomy bag.
- Changes simple non-sterile dressing.
- Assists with the use of devices used as aids to daily living for the disabled.
- Assists client with prescribed exercises about which the home support aide has been taught by appropriate professional personnel.
- Performs other tasks permissible under state scope of practice and regulatory guidelines under the direct supervision of an RN.
- Reminds client to take medications, and observes client taking medications.
- If working on a “flex” basis, submits a completed “Availability” form when requested, and honors the commitment to work when assigned within that availability.
- Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Able to read and write English in order to understand and carry out directions and instructions and maintain simple records. Has recent experience in personal care and homemaking. Demonstrates a willingness to learn and a basic understanding of the duties of a home support aide and the needs of the client. Able to accept and use supervision.
- Education and/or Experience: Possess a current Certified Nursing Assistant license.
